Please take a moment to review the following information before completing your registration:

• No charges at registration: You will not be charged at the time of registration. Your first payment will be processed either at your first appointment or on your chosen billing date, whichever comes first.
• Registration fee: Your first payment will include a $100 registration fee per patient, with a maximum of $200 per family.
• Scheduling: New patients are typically scheduled within one week of registration.
• Insurance: We do not bill insurance for memberships.
• Disability forms: Please note that no disability forms of any kind will be completed within the first 90 days of becoming a patient.

Once registration is complete, the provider you selected will reach out by the end of the business day via text message. Please reply to that message to confirm and schedule your first visit.
