Become a Family Tree Primary Care Member

We’re honored that you’re considering Family Tree Primary Care for your healthcare needs. To ensure every patient receives the personalized care and access they deserve, we are currently using a New Patient Prescreening Form before sending enrollment invitations. This allows us to match new patients with the next available provider while maintaining the high-quality, relationship-based care our practice is known for.

Before You Begin

Please review the following information before submitting your prescreening form:

  • No charges today: There is no cost to complete the prescreening form. Membership fees and registration fees are not charged until your enrollment has been completed.
  • Registration Fee: Your first payment will include a $100 registration fee per patient, with a maximum of $200 per family.
  • Provider Availability: As providers become available, we’ll send you a secure enrollment link to complete your membership.
  • Insurance: Family Tree Primary Care does not bill insurance for membership fees.
  • Disability Forms: Please note that disability forms of any kind will not be completed during the first 90 days of becoming a patient.

What Happens Next?

  1. Complete the New Patient Prescreening Form.
  2. Our team will review your request.
  3. When an opening becomes available, we’ll email you a secure enrollment
    link.
  4. You’ll have 48 hours to complete your enrollment so we can reserve your
    spot.
  5. Once enrolled, your provider will contact you within 1–2 business days by
    text message to schedule your first appointment.
