At Family Tree Primary Care, school and sports physicals are included with their membership. Patients can schedule an appointment to have forms completed with the same care team that already knows and supports their ongoing health needs.

We offer discounted child membership pricing for patients ages 0–18 with an enrolled parent, making family care more accessible and straightforward.
Not every concern requires an office visit. Families also have direct access to their provider by phone, text, email, or telehealth when appropriate. When your child does need to be seen in person, our Direct Primary Care (DPC) model allows for longer appointments with more personalized attention and less rushed care.
