Medications are one part of your overall care plan. We make time to review what you’re taking, how it is working, any side effects or concerns you may have, and whether changes may be appropriate.

Just as importantly, we want you to understand what you’re taking and why, so you can feel more confident about your treatment plan.

Monitoring & Follow-Up

Managing a chronic condition often means paying attention to patterns over time. Depending on your health needs, your provider may recommend follow-up visits, blood pressure checks, lab testing, EKGs, preventive screenings, or other monitoring. Many primary care services are included with your membership. Medications, lab work, and imaging are separate from membership, but Family Tree helps keep those costs affordable through our in-house medication program and contracted cash pricing with lab and imaging partners. Some in-office procedures are included with membership, while others may have an additional cost. We’ll always let you know about additional costs upfront.

When Your Care Needs Change

Even a condition that has been stable for years can change.

Maybe your numbers start trending differently. A medication is not working the way it used to. You notice a new symptom.
