Chronic Condition Management
Chronic Care
Care That Keeps Up With You
Living with a chronic condition usually means your health changes over time. Your care should be able to change with it.
At Family Tree Primary Care, chronic condition management is built around an ongoing relationship with a provider who gets to know your health, your history, and what matters to you. Instead of limiting your care to a short appointment every few months, we make time to talk through symptoms, medications, lab results, lifestyle changes, and the questions that come up along the way.
The goal is to help you understand what is happening, stay on top of changes, and make informed decisions about your health with a team you know and trust.
Why Family Tree?
Chronic Care, The Family Tree Way
A chronic condition is only one part of your health.
Your medications, sleep, stress, activity, weight, family history, other diagnoses, and everyday routines can all affect how you feel. Having an ongoing relationship with your primary care provider makes it easier to look at the whole picture instead of focusing on one symptom or one number at a time.
A Provider Who Knows Your History
Spend less time starting your story over. Your dedicated provider gets to know your health history, your goals, and what you have already tried.
More Time To Talk
Chronic conditions often involve several moving pieces. Longer visits give you room to ask questions, review results, talk through concerns, and understand the reasoning behind your care plan.
Support Between Visits
Health questions do not always wait for your next appointment. Family Tree members can reach their care team directly by phone, text, email, or telehealth when guidance is needed.

Medication Review & Management
Medications are one part of your overall care plan. We make time to review what you’re taking, how it is working, any side effects or concerns you may have, and whether changes may be appropriate.
Just as importantly, we want you to understand what you’re taking and why, so you can feel more confident about your treatment plan.
Monitoring & Follow-Up
When Your Care Needs Change
Even a condition that has been stable for years can change.
Maybe your numbers start trending differently. A medication is not working the way it used to. You notice a new symptom.
Because your provider knows your health history, your usual patterns, and what matters to you, those changes have more context. You don’t have to wait months for your next routine visit just to ask.
Reach out to your Family Tree care team. We can help you decide what makes sense next, whether that means answering a question, scheduling a visit, reviewing your treatment plan, ordering additional testing, or adjusting your care as your needs evolve.
If additional expertise is needed, your Family Tree provider can also help coordinate care with a specialist. Family Tree remains your primary care home base, helping keep medications, recommendations, and the different pieces of your care connected while continuing to look after your overall health.
Chronic Care That Feels Personal Again
Living with an ongoing health condition already takes attention. Getting the primary care you need should feel simple, personal and supportive.
At Family Tree Primary Care, you have more time with your provider, easier access when questions come up, and ongoing support as your health needs change.

Common Questions About Chronic Condition Management
Yes. Ongoing primary care for chronic health conditions is included with your Family Tree membership. That can include follow-up visits, medication management, preventive care, and direct communication with your care team.
Medications, lab work, and imaging are separate from membership, but members have access to affordable options through Family Tree’s in-house medication program and contracted cash pricing for labs and imaging.
We commonly help patients manage high blood pressure, diabetes and prediabetes, high cholesterol, thyroid conditions, asthma, weight and metabolic health concerns, anxiety, depression, and other ongoing health needs. If your condition requires specialty care, we can help coordinate the next step.
For conditions we manage, your provider can review medications, discuss side effects or concerns, make adjustments when appropriate, and help with ongoing refills. We can also review medications prescribed by specialists so your primary care plan stays connected.
Medications, lab work, and imaging are separate from your membership. Family Tree helps keep those costs affordable through our in-house medication program and contracted cash pricing with lab and imaging partners. Some in-office procedures are included with membership, while others have an additional cost. We’ll always let you know about any additional costs upfront.
That depends on your condition, your treatment plan, and how you are doing. Because Family Tree membership includes office visits, your provider can recommend follow-up based on your health needs rather than trying to fit everything into one annual appointment.
Reach out. Members have direct access to their care team by phone, text, email, or telehealth. Some questions can be handled without an office visit, while others may be better addressed in person. We can help you decide what makes sense.
Absolutely. Many patients receive care from both Family Tree and specialists. Your Family Tree provider can remain your primary care home base, helping coordinate recommendations, review medications, and keep the different pieces of your care connected while continuing to manage your overall health.
Yes. In fact, this is one of the benefits of having a primary care provider who knows your broader health history. Instead of looking at each condition in isolation, we can consider how your diagnoses, medications, symptoms, lifestyle, and goals fit together.
